Let’s get this so you can get your record available to you.

Three steps:

Step 1.  Create a User account for username, password, payment, and that kind of stuff.

Step 2. Provide information about the patient, which is usually yourself.  This will give us permission to collect and store your record.  

Step 3.  Provide us a list of the providers you want us to collect your records from.  We need the practice name, not the doctor’s name.  Doctors will change clinics sometimes, and your record doesn’t follow the doctor, it stays with the practice they were in when you saw them.

First we are going to have create an account.  That includes username, password, basic information about the user, and getting payment taken care of.
